Improving the Cancer Journey

The Dundee Macmillan Improving the Cancer Journey service is a partnership between Macmillan Cancer Support and the Dundee Health and Social Care Partnership.

The  service provides seamless and accessible practical and personal support for people affected by cancer, based on their individual needs and what matters to them, so that they can live as well and as independently as possible.

  • We invite people with a cancer diagnosis to have a discussion about their needs and have an individual action plan developed around them
  • Provide the dedicated support of a named professional worker to everyone who uses the service
  • Develop support solutions beyond clinical treatment to ensure health and wellbeing is maximised
  • Build on existing links in local communities and making best use of available resources and partnerships
  • Encourage self-directed support and self-management.
