Tell us your views about future priorities for health and social care in Dundee - complete our survey

The Dundee Health and Social Care Partnership (the ‘Partnership’) is responsible for delivering personalised health and social care services/supports to adults in Dundee.

The Partnership must have a Strategic and Commissioning Plan which describes its vision and priorities for health and social care supports and services. This plan includes information about how we plan to deliver our vision and priorities, as well as the difference this will make to people. The Partnership’s first plan comes to an end this year and we are preparing a replacement plan for 2019 – 2022.

The Review builds on what has been achieved by the Partnership over the last three years and describes strategic priorities for the next three years to ensure Dundee people get the right information and support at the right time. The draft plan has been informed by our ongoing conversations with individuals and communities about their health and social care needs and about how supports and services should be designed and delivered in the future.

We have written a draft plan and want to know if people who use health and social care supports and services, their families, communities and carers think this plan will meet their needs. We will listen to what people tell us before we produce the final Strategic and Commissioning Plan for 2019-2022.

We have created a short survey that we would like you to complete to tell us what you think about the draft plan. If you would like to give us your views you can complete the survey online here. This should not take more than 10 minutes.

If you would like to request either the draft Strategic and Commissioning Plan or online survey questions in an alternative format please contact dundeehscp@dundeecity.gov.uk

Views can be provided up until 22nd February 2019
