Homes from Hospital is a project that has been set up to support older people to make the transition from hospital to home. Dundee Voluntary Action and the Dundee Third Sector Interface have developed this project in partnership and collaboration with local communites so as to ensure that the service adopted a genuine co-production approach throughout.
Home from Hospital is a friendly and confidential service. It makes use of trained colunteers who provide one-to-one short-term practical support. This support enables patients to return home with confidence when no other support is available from friends or family. Volunteers can help with shopping, collecting prescriptions, looking after pets, or just being there when people begin to do things for themselves.
